With a diverse range of backgrounds and specializations, our team works collaboratively to address a wide variety of health concerns, tailoring care to each patient’s unique needs. Below, we introduce our esteemed providers, each playing a pivotal role in fulfilling our mission of delivering excellent health services to the communities we serve.


Earned his medical degree at the University of California, Irvine, where he trained in general surgery and in urologic surgery at Brigham and Women’s Hospital in Boston before changing fields to general practice. Dr. Rosenberg has a special interest in the medical management of urologic diseases and has authored or coauthored articles appearing in Urology, Journal of Urology, BJU International, International Journal of Clinical Practice, and other peer-reviewed journals. He now practices in Jackson, Michigan, serving as Medical Director of Mid-Michigan Health Centers, and on staff at Allegiance Health, where he served as Chief of the Department of Family Medicine from 2003 to 2006. Dr. Rosenberg is Section Editor of Urology at the International Journal of Clinical Practice and is Founder and Chairman of the Urologic Health Foundation, a nonprofit group dedicated to the education of primary care physicians in the field of genitourinary disease. In 2011 he was appointed by the AUA Office of Education to be the Coordinator of Primary Care Education.

Stephen Arntz, MD

Earned his medical degree from Wayne State University School of Medicine and completed his residency training at Oakwood Hospital in Dearborn, MI. Dr. Arntz served for 4 years in the United States Navy at the National Navy Medical Center in Bethesda, MD. He is a member of the Michigan State Medical Society and the American Academy of Family Physicians.

Channing Smith, MD

Earned his medical degree from Michigan State University School of Human Medicine and completed his Family Medicine residency at Miami Valley Hospital in Dayton, Ohio. He served with the United States Army for 4 years at Fort Leavenworth, Kansas. Dr. Smith is a member of the American Academy of Family Physicians and the Michigan State Medical Society.

Elizabeth Zwick, PA-C

Earned her Masters of Physician Assistant Studies from Grand Valley State University. She is a native of Jackson and graduated from Jackson High School. Liz and her husband returned to Jackson in May of 2010 shortly after graduation to begin working with Mid-Michigan Health Centers. She is a member of the American Academy of Physician Assistants as well as the Michigan Academy of Physician Assistants.

Lezlie Malson, FNP-C

Earned her Masters of Science in Nursing from Maryville University of Saint Louis and began working with Mid-Michigan Health Centers in June 2015.

Brandon Douglass, PA-C

Earned his Masters of Physician Assistant Studies from Kettering College of Medical Arts. He is a native of Somerset Center and graduated from Addison High School. He currently lives in Jackson with his wife and daughter.

Jasmine Rankin-Hawkins, PA-C

Earned her Masters of Physician Assistant Studies from Western Michigan University and began working at Mid-Michigan Health Centers in 2020.

Audrey Baxter, RDN

Earned her Bachelors of Science degree in Dietetics from Michigan State University. She is passionate about assisting others in improving their quality of life through nutrition. Audrey provides one-on-one individualized care to clients for weight management, diabetes self-management and more.
